Should the documents to be submitted along with the form be self attested or do we need to get them attested by someone? If yes, by whom?

It would be better if they're attested by a CA or MLA. Though, it won't make any difference if you yourself attest it. -
You can submit all the scores whatever you have appeared at or wherever you have scored highest. Class 10th, 12th certificates, graduation mark sheet and certificate. If applying through reservation quota then caste certificate and domicile certificate. Yes, all the documents have to be signed by the government officer, retrieved army officer, school principal and Dean of the college.

You can get it attested from any gazetted officer i.e. executive, managerial and supervisorial level ranked public servants in India.
