IELTS stands for International English Language Testing System, It is an international standardised test of English language proficiency. IELTS is accepted by most Australian, British, Canadian, Irish, New Zealand and South African academic institutions, over 3,000 academic institutions in the United States, and various professional organisations. It is also a requirement for immigration to Australia, New Zealand and Canada. When you take the test and the better you score, the institution would take it that you are a better user of the Language. As such just by taking an IELTS exam without any other reason would not be of any help to you. as mentioned earlier, if you are planning for further studies/Immigration etc then, you would have to and it is preferred that you give such an examination. There are other similar exams for English too. PTE,TOEFL,BEC,TOEIC etc etc

Marketing is the safest specialization but you can consider very contemporary and high potential specializations like Supply Chain and Operations Management provided the institute you are planning to join have it or the they can really teach it as many offering many things but cannot teach them. MCA has nothing to clash with your MBA as you need to MBA in absence of a management qualification with whatever other UG/ PG degrees you have. If you have self confident, passion and zeal to do something in life then 'systems' or 'IT' can also be a great MBA specialization but once again make sure that the institution you plan to join they can really teach it ! Before you finalize about your B-school please check their credentials from the alumni and current senior students as all B-schools claim to be great and in real life they are not.

Pursuing MBA abroad is always a good option, the main reason for seeking a MBA degree from abroad are:
