Please remember as the medium of education in American universities is english so all the international students whose first or native language is not english have to take TOEFL test to prove their english language proficiency.
So if your first or native language is not englihs in that case you have surely take TOEFL test to get admission in American universities to pursue MS program.

No, I donlt think any University demands an Affidavit from a candidate certifying that he/she has read all the rules & regulations. But yes, there would a form which mentions the rules and regulations and the candidate needs to sign it with a statement that he/she has read them, understood them and agree with them.
