The eligibility criteria for Ph.D varies for every institute. However the basic requirement is a masters degree or an equivalent degree in the relevant discipline.
However the cut off percentages vary. Some of the institutes conduct entrance exams and some of them require atleast two research publications for the Ph.D programs. Some also require work experience of a couple of years.

The current selection process of IIMs takes into consideration a lot many factors like
1. CAT Percentile and GDPI performance
2. Academic qualifications as well as percentages
3. Work-experience (Optional)
4. Extra-curricular and co-curricular achievements
5. Certifications etc
Having work-ex or CA/CS or a degree from IIT will definitely increase ones chances of getting into an IIM, but not mandatory.
