If you want to study in USA in prestigious institutions, you need to have the strongest profile among thousands of applicants. I mean 1600 out of 1600 on SAT and 110 above on TOEFL and still no one can assure you admission in these institutions.
After this holistic approach, if you get in Ivy Leagues, they will offer you blind scholarships, which is similar to free. But, you must look at the admission rate of each Ivy League you want to apply.

If you are asking about this specific branch then you must either be very much interested in this field or you must be taking it due to some other reason. First of all, I would suggest that opt for this only if you are personally very much interested in it and happen to know about something / someone of this field. A career in it would be tough and may take some time to strengthen up your requirements. But yes the good point is that you will have lesser competition.

If a fresh graduate works in private clinic or hospital, they may get 25,000 to 40,000 per month. If they open own practice, the income can be different for each person. In government jobs, salary is more fixed and comes with extra benefits.

You can go for operations, if you are strong in mathematics. As you are from engineering background, I would suggest you to go for operations, else you can also go for HR or marketing.

Yes, you class 12th percentage do matter, a minimum of 75% is needed to crack IIT JEE advanced. Other entrance exams do have eligibility criteria of around 60%.
