Hello Parth,
After IITs, the best choice for engineering is obviously BITS Pilani. You need to write BITSAT to get admission into it.
Then, you can opt for VIT University (Chennai and Vellore), Thapar University, PSG Coimbatore, Sastra University, Manipal University, SRM University, Amrita University, etc.

If you want to go in Medical field, then you should opt for Doctor or Pharmacy. After the degree of pharmacy (B. Pharmacy), you could run a Medical or become a Drug inspector etc. But, rather than asking others, search your preferred way. It would be better if you search on internet about Career after PCB 10+2.

Pavan, Automobile engineering is a branch of Mechanical engineering, where you'll deal mostly with automobile, whereas in Mechanical engineering, you'll deal with all machines and its design and then many more basics. But literally both are same, no big difference. But better option is to go for Mechanical as its scope is more and wide.
