What extra documents is required for the candidates applying under PWD category in JEE Advanced?

<p>The PWD category candidates need the below given documets to apply for JEE Advanced:</p><ul><li>Physical Disability certificate (Unique Disability Identification (UDID)&nbsp;Certificate or FORM-PwD (II)/FORM-PwD (III)/FORM-PwD (IV), &nbsp;whichever is applicable) issued by the notified medical authority.</li><li>Dyslexic candidates need to submit FORM-DYSLEXIC-1 and FORMDYSLEXIC-2 instead of FORM-PwD. The certificate must mention&nbsp;'SEVERE' under Dyslexia category to enable the candidate to get the benefit&nbsp;of PwD category.</li><li>Individual IITs constitute medical boards at the time of admission to test the&nbsp;candidate's level of disability, where the candidate has not submitted the&nbsp;UDID certificate.</li></ul><p>Other than the above, the below given documents shall be also needed:</p><ul><li>Class X certificate if the date of birth is mentioned in it or birth certificate.</li><li>Class XII (or equivalent examination) certificate [for those who appeared first&nbsp;time for this examination in 2024 or 2025 or whose Board declared the Class XII (or equivalent examination) result of academic session 2022-23 on or after June 28, 2023]. If the marksheet of Class XII (or equivalent examination) for 2025 is available, then it must be uploaded.</li><li>If the candidate's name is not same as in the Class X certificate, gazette&nbsp;notification showing the change of name.</li></ul>
