For that to happen, your application and persona should match the requirements of specific B-schools. If I talk about IIM -A they give 70% weightage to CAT score and remaining 30% to your application rating score.
The ranking and final selection is based on a diverse set of attributes which include overall performance in CAT/GMAT and also performance in individual sections, performance in personal interview, academic, co-curricular and extra-curricular achivements, work experiance etc.

To get admission into top B-schools you need to have atleast more than 60% in your gradation and more than 90 percentile in the MBA entrance exam. Internship and placements are based on your MBA score/performance in all the semester exams. Some companies do consider your graduation marks also.

Usually migration is permitted on a case to case basis within the university itself in the second year. However, migration across institutes after the second year will not be permitted. However, do check with your current college on the guidelines.
