You cannot compare these two examinations. If you are keen to work with Indian Army as a doctor, then go for AFMC Otherwise, NEET. Both have different needs, like after NEET, you would be working with the local public directly. On the other hand, after AFMC, you would be attached to different Army units across the nation and work with them.

Yes, you need to pay each and every time the examination fees for appearing in the TOEFL exam. And it is applicable for the other exams as well.

You must take the GRE and have a good GRE score. Top universities are highly selective and therefore the top ones. 64% is not enough so you must bank on your GRE score, work experience, SOP, LOR, TOEFL score and overall profile.
