First of all transfer system is not very common in Indian Universities, moreover as, you are planning to get a transfer from Distance education to regular it becomes more difficult, In case you take a transfer from regular to Distance programme there is still a possibility but from Distance education to Regular the chances are very thin.
However, I would suggest you to get the most appropriate information you should contact the University itself.

Once you have fulfilled the above requirements, you would be eligible to apply to any top university in USA. Some of the top medical colleges in USA are Harvard University, Washington University in St. Louis, Duke University and many more.
Fees depending from university to university and location to location. On an average, you should assume the fees to be 60,000 USD per annum including living expenses.

As fashion designing is a skill oriented & a very creative field so if you have the required skill sets you definitely can make a successful career in it.
Fashion designing is a field in which one needs to be artistic and creative, should be able to express his/her ideas in sketches, may not be brilliant artists but must be able to combine colors, shades, and tones should have the flair for playing with fabric and know how to use textiles effectively. Designers need to have visual imagination and the ability to think in multi-dimension to translate into garments what they visualize. He/she should be fashion conscious and must be aware of market requirements. An understanding of the market and customer lifestyle is required, so that designs are suitable for people they are made for. Designers must be good communicator and very clear with their ideas which is reflected in the presentation of fashion shows. So, if you feel you fit the bill then go ahead.
As far as NIFT is concerned, it is one of the well known institutes in fashion designing Industry, so if you are planning to get into NIFT, I would say go ahead.

Although, you can go for jobs in railways but it would depend on your qualification that which department you join. If you are from Engineering (Civil / Mechanical / Electrical /electronics etc.) background you can go for jobs like Section Research Engineer, Section Engineer etc. or else you can go for administrative jobs like station masters, traffic Apprentice, stenographer etc. for all such positions you have to apply through railway recruitment board and appear for Entrance Examination conducted by them. Apart from this you can enter Railways through UPSC Examination for various positions in Traffic services etc.
