Hi,
Banks do not provide full loans, they offer 70% to 80%. Admission process differs with universities. There is not one criteria for selection, the admission decision depends on the overall application package of the applicant. Every university's selection criteria also differs. Take the GRE, if you wish to apply to universities in US, since your academics is not so high you must secure a very good GRE score - something above 315. Additionally, you must take IELTS or TOEFL and score well enough. Take help of an overseas education consultancy, if you need assistance in this regard.

I would suggest you to first identify your area of interest. This is because there are many courses you can opt for depending upon your interest. For instance, if you have interest in cost accounting, then you can go for CMA, if corporate law interests you, you may opt for company secretary course. Furthermore, you can also have a look at executive MBA since you have more than 5 years of experience. In my opinion, career choice should not solely depend upon income prospects. Moving into a career that interests you will give you much more satisfaction.

I would also suggest you to be a certified trainer in say soft skills especially so business communication and leadership, development and all. BHMS has a better scope.
After completing BHMS, you can do the following:
MD is Masters Degree level.
Ph.D is Doctoral Degree level.
The best option is to pursue both
However, you should watch ICMR website for announcement of Junior Research Fellowship(JRF) - Rs. 16000/- per month, & Senior Research Fellowship(SRF) - Rs. 24,000/- per month approx, for your PG and Ph.D. studies respectively. Also you must not forget to tick yes in the box for "Lectureship also".
You need this lectureship to apply for Assistant Professors & Associate Professors in government medical colleges.
Your pay packet at end of the first month itself will be about Rs. 48
