You must write to the university regarding IELTS or any other requirement, they may ask for a GRE too. Tuition fee for international students may range from $8000 to $15000 a year. You must check the tuition rates on university websites, if it includes health coverage and additional student fee. As for Ph.D admissions, you can get in touch with the department specific advisors, enquire about stipends, assistantship, scholarships, international student awards, etc.

If you want to go for fashion designing, there are some colleges which offer fashion design course. Some of the best colleges for fashion design are NIFT, NID and pearl academy.
