In order to pursue undergraduate degree from universities abroad, you need to first of all shortlist the universities you wish to apply.
Once shortlisted you can contact the admission department of the (either through phone or email) university, and find the eligibility criteria to pursue undergraduate degree.

Good academic record, preferably 60%-70% plus in 12th.
Relevant 12th degree from recognize education board.
SAT score of 1700-1800 plus (Required by universities in US)

'How to prepare' is a question which may vary from one person to another and it may not be necessary that what is advised can be thoroughly implemented as the capabilities, preferences and academic records vary individually.

* Next would be the curriculum. Make sure that you have all reference texts covering your syllabus well, also the sample, mock and previous test papers.
* Time table - Following a proper study plan is very important and should be followed strictly.
* Self Analysis- solving more and more mock tests will help you realize your area of strengths and weaknesses, you may then accordingly work on the same.
* Lastly, it is very important that you display self confidence, perseverance.
Other than this, you can also take some support from a good test prep institute which will help you with the preparation well as these institutes are equipped with latest information about the latest patterns and changes of an entrance.
