The difference between NEET and JIPMER is speed. JIPMER requires you to be quicker than NEET. NEET is a very balanced exam, where speed, concepts and presence of mind is tested. AIIMS is a whole different exam.

It's up to you. But pursuing B.Com from top colleges has some advantages than from hometown. Top colleges will allow you to change your attitudes, thinking and strengthen your mental power and motivated to study well. And give more knowledge from highly educated professors, will help you become self-sufficient too and you will get placements after graduation. But if you take admission in a college located at in your hometown, you will enjoy the campus life without concentrating on studies. You won't be able to give much thought to your career.
