You should join a good institute. other than that start reading english newspaper, story books of your interest. try to listen english news channels and write up your progress, experience everyday. check and rectify grammatical mistakes. most importantly, speak in english with brother, parents. if this is not possible, try to speak in english infront of mirror. it will help u to boost up your confidence level and pronounce the words clearly and firmly. all the best. u can do it. thank you.

If you want to do MBBS or BDS, you have to appear AIPMT.
Other than these, you can go for BSc Medical Science, BSc Nursing, BPharm, BHMS, BAMS.
I personally recommend you to appear AIPMT for MBBS BDS
