There is no point for you to take the TOEFL right now as the validity for the exam in only 2 years. You may take the GRE this year as its validy is 5 years but it would be pointless to take the exam right now as it would be off no use to you. I would recommend you to finish your 3rd year first and then start preparing and take atleast 6-8 months to prepare for both the exams and finally give the exams once you awaiting your 4 year final semester result. In this way you would give yourself time to prepare also, as well as you would take the exams right before you would be applying to the universities for your masters.

If you want to pursue engineering in future, then you should focus on PCM only. Though you can opt for Biology as an additional subject, but unless you are aiming for medical line, it will be of no use.
Also to say about the scores, be it engineering or medical, all depends on the performance of yours in the respective entrance exams and not on the percentage.
Normally minimum of 60-65% is sufficient and you seem to be a good student with 89% in matriculation, so don't worry for marks.
Still i would suggest you to focus on one stream and aim for exams of that streams only as otherwise you may end up with average scores i both,
