Cabin crew is made up of flight attendants as well as the purser, one who is typically a senior flight attendant but also has additional duties. Some airlines make a bigger deal about the purser and that this is a title or rank above "flight attendant". Majority of US colleges require a MS degree for direct Ph.D admission. This will also depend on the line of work you intend to research and entirely on the university requirement. There are certain programmes and universities such as MIT which do not require masters and you can go to Ph.D directly after graduation. Certain programmes will however require you to enroll into the integrated Ph.D (MS/Ph.d).
You may check MIT, UMass Amherst, Caltech and CMU for the same.

Yes, all airlines domestic or international have their individual training centers. Primarily these centers are used for training the new recruitment in cabin crew.The other departments such as reservation, ground staff etc generally have their on desk training.
The training centers are required top introduce the cabin crew to the anatomy of aircraft, process of airlines, inform them of airline facility, brief them about safety procedure, train them on First aid and also brief them on the additional duties on board such as inflight shopping facilities etc.
