IT is a field which involves a lot of technology and the same tends to change with time passing, hence one always requires to update self on the latest technologies.
There are many types of courses that one can pursue depending on his/ her interest and to be able to provide you information about the same you may first revert with your interest intent i.e. whether you plan to take up some other language course like Java, or want to change your stream and would want to get in to fields like hardware/ Networking or would prefer courses like SAP etc.
There are many courses but without knowing your interest it is not possible to provide you with any specific information.

There are many options that you can look upon and depending on your interest you may then look forward to pursue a one among these. You can opt for a Masters degree as this will definitely provide you better clarity, and bring in brighter career prospects.
Other than this you can also take up other international certifications like Red Hat which are highly [popular and in demand. Also if you are keen to continue on CISCO, I would advise you to upgrade yourself by taking up other certification like CCNP as CCNA is a simple basic certification.

It is mandatory to give the TOEFL. You can submit an application without the TOEFL score and apply for a TOEFL waiver but if you do not get the waiver, you would have to take the TOEFL. In any case, you would be required to take the TOEFL for the visa. So it is recommended that you take the TOEFL anyway.
I would also like to let you know that MBA requires you to have 2 years of Full Time work experience atleast. Therefore, if you do not have work experience, i would recommend dropping the idea of pursuing MBA for the time being and concentrate on your work experience.
