Choice is better but you are pursuing IT. So, You can please complete the IT and then pursue your interest in Medical.

Singapore's NTU has a course which costs about 15 lakhs in tuition for one year. USA has many options but would be costlier than Singapore.

Yes, you can get it obviously but before that give your compartment exam and clear mathematics with a decent score, at least with passing marks.

No Doubt b.tech! after that m.tech or MBA. Degree has weightage which diploma doesn't. still it's upto you completely.your field of interest :)

TOEFL 91 marks is fine however the number of backlogs is a lot for any or at least many universities in Canada. Are you wanting to do a PhD after your Masters? If not, then look into the job-oriented SPP PG Diploma programs offered by SPP colleges and university colleges which sometimes incorporate a paid internship. Hope this helps. Thanks!
